Official transcripts of your academic record and duplicate grade reports can be requested in writing from the Office of the Registrar at the Davis Campus or Trafalgar Road Campus. There is no charge for this service. Students can also select from the list of online forms below to request or change certain information. All forms are official and require student number, birth date and originating email address to authenticate your request. These items form the equivalent of your "electronic signature" and will be treated as if a signed form was submitted to the Registrar's Office.
